Abdominal Wall Surgery: from diagnosis to repair
Abdominal wall surgery is the branch of general surgery that repairs defects of the wall: hernias, incisional hernias and rectus diastasis. This site, authored by Dr. Federico Fiori, explains anatomy, classifications, symptoms, investigations and repair techniques.
Abdominal wall surgery is now a super-specialized branch of general surgery. A thorough knowledge of the anatomy and a precise understanding of the pathophysiology make it possible to approach this very heterogeneous field with different operative techniques, chosen to get the best result for the patient. Defects range from small umbilical hernias to the hostile abdomen that calls for complex abdominal wall reconstruction, and the options span open, laparoscopic and endoscopic surgery through to the robotic approach. As always, it is the correct indication that makes the good surgeon and determines the result.
For this reason it helps to set out which defects affect the abdominal wall, why imaging matters, which surgical options exist, and the role of pre- and rehabilitation and of the related treatments that improve the overall outcome. This site focuses on the anterior abdominal wall and leaves aside the groin region, for which dedicated resources already exist, touching on it only to present some minimally invasive techniques.

What is this site about?
This is the English vertical site on abdominal wall surgery by Dr. Federico Fiori: it explains the anatomy of the wall, the classification of defects, the symptoms, the investigations and the repair techniques, with the surgeon's clinical text and the educational articles of the federicofiori.com editorial network.

Each topic in the network has its own reference site: rectus diastasis is covered in full on diastasisandbeyond.com, while the curriculum, the TESAR technique, the MILA technique and the glossary live on the federicofiori.com hub. Everything you need to understand hernias and incisional hernias, and to come prepared to the consultation, stays here.
Hernias and primary wall defects
A hernia is the protrusion of a viscus or tissue through a weak point in the abdominal wall: epigastric, umbilical or Spigelian. Locations, labels and sizes are on the page about the classification of abdominal wall defects.
Incisional hernia: the hernia on a scar
An incisional hernia is the defect that appears on the scar of a previous abdominal operation. How to recognise it and when it becomes urgent is explained on the symptoms and complications page.
Prehabilitation before surgery
Prehabilitation is the clinical preparation in the weeks before surgery: weight, blood glucose, smoking, nutrition and exercise. The path is described on the patient prehabilitation page.
How do you get to surgery?
- The specialist assessment in the clinic, with the clinical examination of the wall: you can book it from the contact page.
- Imaging (ultrasound, CT or MRI) when the defect needs to be measured: the investigations are described on the abdominal wall imaging page.
- Prehabilitation, when the picture calls for it, to reach surgery in the best possible condition.
- The operation with the technique chosen for the individual patient: the techniques are on the surgical treatment page.
If you live in Italy without speaking Italian, or you are planning to come from abroad, the page for patients looking for an English-speaking hernia surgeon in Milan explains how the consultation works, in English.

Why is the wall assessed together with the shape of the abdomen?

In the abdominal wall, structure and appearance are not two separate problems: they are the same structure seen from two sides. A defect that is well closed but leaves an abdomen the patient does not recognise is only half a result, and a correct profile over a wall that remains weak does not last. For this reason the assessment keeps three things together: the musculo-aponeurotic defect, the covering tissues and the person's everyday movements.
This is the criterion behind the indications described on these pages. The method, with its principles and its limits, is explained in full on the network hub: federicofiori.com.
